So you look at the adnexa to see where the free fluid is coming from. The DDx now is

1.  ruptured corpus luteum cyst

2. ruptured ovarian cyst

3. ovarian torsion

4. intestinal pathology eg appendicitis

This is what you see:

So in this case, this pregnant female with sudden onset RIF pain and pelvic FF, had a ruptured corpus luteum cyst causing haemorrhage.
